Young People's Support Facilitator

We are seeking a dedicated and compassionate Support Worker to join our team at Horizon Care & Education. As a Support Worker, you will play a vital role in supporting unaccompanied asylum seeking children and care leavers to live independently.
Key Responsibilities- Provide practical and emotional support to young people to help them achieve their goals and live independently.
- Work collaboratively with colleagues to ensure a high-quality service is delivered to our clients.
- Develop and maintain relationships with young people, their families, and other stakeholders to ensure a comprehensive support network.
- Contribute to the development and implementation of individual support plans to meet the needs of our clients.
- Participate in ongoing training and professional development to enhance your skills and knowledge.
