About the Role

We are seeking a highly motivated and compassionate Support Coordinator to join our team at Home Group Limited. As a Support Coordinator, you will play a vital role in providing person-centered support to individuals with complex mental health needs, enabling them to develop the skills and confidence to live more independently in the community.

Key Responsibilities
  • Creating and implementing personalized support plans with customers, working closely with clinicians to develop risk management plans.
  • Supporting customers with independent living skills, such as budgeting, maximizing income, cooking, and engaging with external services.
  • Promoting Home Group's values within the team and working collaboratively with local authorities, NHS mental health teams, safeguarding teams, and housing option teams.
