About the Role

We are seeking a skilled and compassionate Community Nurse to join our team at Sirona care & health CIC. As a Community Nurse, you will work in a variety of settings, including patients' homes and community centers, providing high-quality care and support to individuals with complex needs.

Key Responsibilities
  • Implement and evaluate effective treatment plans for patients with complex nursing needs
  • Work collaboratively with multidisciplinary teams to provide holistic care and support
  • Conduct basic holistic assessments in a clinical setting
  • Develop and maintain strong relationships with patients, families, and carers
  • Participate in training and professional development opportunities to enhance skills and knowledge
Requirements
  • BSc or equivalent experience in adult nursing
  • Current NMC registration
  • Knowledge of the principles of Clinical Governance and its applications
  • Up to date knowledge of Evidence Based Practice, local and national standards
  • Community Nursing Experience
