Job Summary

We are seeking a dedicated and compassionate Support Worker to join our team at Libertatem Healthcare Group. In this role, you will provide high-quality care and support to individuals with complex health needs, ensuring their comfort, safety, and well-being.

Key Responsibilities
  • Deliver personalised care and support to clients with complex health conditions, promoting independence and dignity.
  • Assist with daily living activities, including personal hygiene, mobility, and meal preparation, ensuring a high level of care and attention to detail.
  • Administer medication and monitor health conditions, maintaining accurate records and reporting any changes to healthcare professionals.
  • Provide emotional support and companionship, building strong relationships with clients and their families.
  • Maintain accurate records of care provided, ensuring compliance with regulatory requirements and best practices.
  • Collaborate with healthcare professionals and family members to ensure comprehensive care, promoting a holistic approach to client well-being.
Requirements
  • Previous experience in a similar role, preferably in complex care, with a strong understanding of client needs and preferences.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and healthcare professionals.
  • Compassionate, patient, and empathetic nature, with a commitment to delivering high-quality care and support.
  • Ability to work independently and as part of a team, with a flexible approach to meeting client needs.
  • Flexibility to work the specified hours, with a willingness to adapt to changing circumstances.
