Role Overview

The successful candidate will be an integral part of a dedicated care team, providing community-based palliative nursing care. This role involves working closely with families who are caring for children and young people facing life-threatening and life-limiting conditions.

Key Responsibilities

  • Deliver high-quality, individualized holistic care and support to children and young people with complex medical needs and their families, fostering empowerment and collaboration.
  • Supervise children and young people in the absence of their parents or guardians.
  • Maintain effective communication with all members of the care team.
  • Promote evidence-based practices that enhance the care provided to children on the caseload, adhering to clinical governance standards.
  • Execute all necessary nursing tasks within the home setting, developing and delivering additional clinical skills as training is acquired while ensuring competency.
  • Manage your own workload effectively during respite visits and office-based responsibilities.
  • Adhere to UHBW policies and Jessie May guidelines.
  • Facilitate communication with colleagues and partner organizations.
  • Collaborate with your line manager to enhance services and improve care quality for children and young people.
  • Ensure compliance with Health and Safety regulations, reporting all incidents to your line manager.

Continued Responsibilities

  • Ensure adherence to UHBW policies and Jessie May guidelines in all procedures.
  • Collaborate with the end-of-life team leader to provide compassionate end-of-life care to children on the caseload.
  • Offer guidance and support to students and other staff members as needed.
  • Engage in the implementation of relevant government directives.
  • Follow emergency protocols for children receiving care.
  • Administer medications as prescribed by qualified medical professionals.
  • Assist with nursing needs in accordance with individualized care plans, promoting independence.
  • Provide empathetic emotional support to children and their families.
  • Maintain accurate records of care and observations in line with UHBW policies.
  • Identify and communicate the need for additional emotional support for families.
  • Recognize the importance of bereavement support and maintain communication with the care team to ensure families receive appropriate assistance.
  • Participate in service evaluations to enhance care delivery.
