Job Summary:
HCRG Staffing is seeking an experienced Social Worker to fill a challenging and rewarding role in our Discharge to Assess team. As a key member of our team, you will work collaboratively with a range of stakeholders to enhance prevention interventions and prevent or delay admission into formal support services.
Key Responsibilities:
- Work across the system to enhance prevention interventions and work to prevent or delay admission into formal support services.
- Work collaboratively with a range of stakeholders, including healthcare professionals, community organizations, and individuals.
- Support the admission avoidance and the discharge of people from acute and community hospitals in a safe and timely way, reducing the risk of delayed transfers of care.
- Develop and implement effective discharge plans that meet the needs of individuals and their families.
- Collaborate with healthcare professionals to ensure seamless transitions of care and reduce hospital readmissions.
- Provide education and support to individuals and their families to promote self-management and independence.
- Contribute to the development and implementation of policies and procedures that promote best practice in discharge to assess.
Requirements:
- Minimum 2 years of experience as a Social Worker in a community or hospital setting.
- Proven experience in discharge to assess and prevention interventions.
- Strong communication and interpersonal skills, with the ability to work effectively with a range of stakeholders.
- Ability to work independently and as part of a team.
- Strong problem-solving and analytical skills, with the ability to think critically and creatively.
- Knowledge of relevant laws, policies, and procedures that impact discharge to assess.
