Reablement Support Worker Job Description

**Job Summary:**

We are seeking a skilled and compassionate Reablement Support Worker to join our team at Service Care Solutions. As a Reablement Support Worker, you will play a vital role in supporting individuals to maintain their independence and wellbeing.

**Key Responsibilities:**

* Develop and implement individual care plans to maximize independence and wellbeing
* Carry out therapeutic activities and skills practice to improve mobility, daily living skills, and confidence
* Encourage and support clients to maintain optimum independence, including decision-making, social contacts, and home safety
* Accurately communicate verbally, in writing, and using IT systems to ensure safe and effective service delivery
* Observe, record, and report changes in individual conditions and circumstances to senior staff

**Requirements:**

* Experience working with adults
* Full driving license and access to own vehicle
* NVQ Level 3 in Health and Social Care or equivalent
