About the Role

Key Responsibilities
  • Provide comprehensive assessments and care planning to ensure the best possible outcomes for service users.
  • Work collaboratively with multidisciplinary teams to deliver recovery-focused care and support to service users and their carers.
  • Act as a champion of social care and safeguarding, actively addressing adversity and supporting adults at risk of social exclusion.
  • Champion equality and diversity, challenging discrimination in all its forms.
  • Develop and maintain effective relationships with service users, carers, and other stakeholders to ensure person-centered care.
Requirements
  • Appropriate professional social work qualification.
  • Registration with Social Work England.
  • Training in principles and application of outcomes-focused assessments and personalisation.
  • Relevant post-qualifying training for social workers.
  • Training and experience in safeguarding.
Desirable Qualifications and Experience
  • Approved Mental Health Professional/Best Interests Assessment Training.
  • Experience of work as Approved Mental Health Professional and/or Best Interests Assessor.
