Key Responsibilities- Conduct comprehensive assessments and reviews for adults across the intermediate care pathway, ensuring high-quality, person-centered care that upholds the dignity and rights of service users.
- Work collaboratively across various settings, including inpatient units, acute hospitals, and community-based care facilities, to ensure seamless and effective care delivery.
- Participate in rotational duties, including working in rehabilitation units during periods of high demand, particularly around discharge planning.
- Deliver high-quality care that meets the needs of service users, working closely with multidisciplinary teams to ensure effective care delivery.
- A qualified social worker with a valid registration with Social Work England.
- A strong understanding of the Care Act and experience in undertaking comprehensive assessments and reviews.
- The ability to work flexibly across different care settings, adapting to the needs of the service.
- Excellent communication skills, with a commitment to collaborative working and supporting positive outcomes for service users.
- A competitive pay rate of up to £36 per hour.
