Position Overview
An exceptional opportunity is available for an Advanced Clinical Specialist to become a vital member of the Integrated Community Team at Dorset HealthCare University NHS Foundation Trust. This role is designed for those who are passionate about delivering high-quality, patient-focused care to elderly individuals with complex health requirements.
The ideal candidate will collaborate with various healthcare sectors, including primary care, secondary care, community services, and social services, to ensure comprehensive, proactive, and responsive care as part of a cohesive, multidisciplinary team.
We are particularly interested in candidates with diverse professional experiences who can conduct advanced clinical evaluations and develop tailored care strategies for older patients facing intricate health and social challenges.
Key Responsibilities
Successful candidates should possess extensive experience in community and/or urgent care at a senior level. They should demonstrate motivation, enthusiasm, and a proven track record of functioning as an independent practitioner.
Preferred qualifications include:
- Current registration with NMC/HCPC
- Relevant professional health degree
- Evidence of recent Continuing Professional Development (CPD), aligning with the Health Education England multi-professional framework for advanced clinical practice.
- Non-Medical Prescribing (NMP) qualification
- Postgraduate Diploma in Advanced Practice or a related specialty
This position is part-time, offering flexible hours per week. The service operates from 8:30 to 18:30, Monday to Friday, with plans for future expansion to include weekend services.
Employment in this role necessitates an Enhanced Disclosure and Barring Service (DBS) check.
