Job Overview

The Transformation Unit is seeking a highly skilled Clinical Nurse Specialist to join our team in providing exceptional care to patients with Acute/Chronic Graft Versus Host Disease (GVHD). As a key member of our multi-disciplinary blood and bone marrow transplant team, you will be responsible for coordinating day unit/clinic based post-transplant care, completing clinical skills tasks, and serving as the primary point of contact for patients and their families.

Main Responsibilities
  • Deliver High-Quality Care: Provide efficient and effective haematology day centre/GVHD clinic and inpatient care for GVHD patients and their families.
  • Assess and Plan Care: Conduct thorough assessments, develop individualized care plans, and evaluate patient outcomes using a holistic approach.
  • Liaise with the AHP Team: Collaborate closely with the Allied Health Professionals team to coordinate care and ensure seamless patient transitions.
  • Provide Education and Support: Offer patients and their families information, education, and psychological support to enhance their overall experience.
  • Enhance Clinical Role: Participate in training and development opportunities to expand your clinical skills, including cannulation, venepuncture, and chemotherapy administration.
  • Cross-Cover and Participate: Cross-cover other team members and participate in the nurse-led late effects clinic to ensure comprehensive patient care.
  • Maintain Accurate Records: Maintain up-to-date nursing records and care plans to ensure continuity of care.
  • Link to Clinical Trials: Collaborate with the clinical trials team to ensure patients have access to relevant research opportunities.
Requirements
  • Qualifications: Registered Nurse with current NMC registration, evidence of Continuing Professional Development (CPD), and a degree-level education.
  • Experience: Relevant post-registration experience in haematology or a related specialty.
  • Personal Qualities: Excellent communication and interpersonal skills, good counselling skills, and a commitment to delivering exceptional patient care.
