Cardiac Rehabilitation Practitioner Opportunity

We are seeking a highly motivated and experienced Cardiac Rehabilitation Practitioner to join our Integrated Croydon Cardiac Rehabilitation Team.

About the Role:

The successful candidate will be responsible for delivering high-quality care to patients with Coronary Heart Disease (CHD) within the hospital and community settings. This will involve working closely with the cardiac rehab team to develop and implement individualized exercise programs, providing education and support to patients and their families, and contributing to the development of new services to meet patient needs.

Key Responsibilities:

  • Deliver exercise programs and education sessions to patients with CHD
  • Work collaboratively with the cardiac rehab team to develop and implement individualized care plans
  • Provide support and education to patients and their families
  • Contribute to the development of new services and initiatives to improve patient outcomes

Requirements:

  • Experience of working with patients with cardiac conditions
  • NVQ level 3 or equivalent knowledge
  • GCSE in English and Maths
  • BACPR physical activity course certification
  • Good IT skills, including Excel
