About the Role

We are seeking an experienced and skilled Administrative Assistant to join our Hospital Discharge Team at Royal Berkshire Hospital. As a key member of our multi-professional team, you will play a vital role in supporting the discharge process for patients transitioning to a community setting.

Key Responsibilities
  • Work closely with patients, ward staff, social workers, and other stakeholders to ensure a seamless discharge process.
  • Coordinate care for patients referred to the Hospital Discharge Team and facilitate clear communication with relevant professionals.
  • Provide administrative support to the team, including managing patient records and coordinating referrals.
  • Develop and maintain effective relationships with key stakeholders, including ward staff, social workers, and community services.
  • Contribute to the development of the Hospital Discharge Team, identifying opportunities for improvement and implementing changes to enhance patient care.
