Job Summary:

We are seeking a highly skilled Mental Health Practitioner to join our Home Treatment Team in Congleton. As a key member of our team, you will be responsible for providing high-quality care and support to individuals with mental health needs.

Key Responsibilities:

  • Work across Urgent Care Mental Health First Response Services, including Crisis Resolution Home Treatment Teams, Liaison Services, Street Triage Services, Liaison and Diversion Services, and the Crisis Triage and Response Service.
  • Provide person-centred care and support to individuals with mental health needs, working in collaboration with other healthcare professionals.
  • Develop and implement effective care plans, ensuring the needs of individuals are met and their goals are achieved.
  • Contribute to the development and implementation of policies and procedures, ensuring best practice is maintained.
  • Participate in multidisciplinary teams, sharing knowledge and expertise to improve patient outcomes.
