Job SummaryWe're seeking a skilled and compassionate Support Worker to join our team, providing support and care to individuals with disabilities in a community-based setting. As a Support Worker, you'll play a vital role in enabling our service users to live independently, make informed choices, and participate in activities that promote their physical and emotional well-being.
Key Responsibilities- Provide person-centered support and care to individuals with disabilities, promoting their independence and autonomy.
- Assist with daily living tasks, such as personal care, medication management, and meal preparation.
- Support service users in accessing community resources, such as education, employment, and leisure activities.
- Collaborate with the multidisciplinary team to develop and implement individualized care plans.
- Monitor and report any changes in service user's condition, ensuring timely interventions and referrals.
- Full UK driving license and access to a vehicle.
- Experience working with individuals with disabilities, preferably in a community-based setting.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and colleagues.
- Ability to work flexibly, including evenings, weekends, and bank holidays.
- Compassionate and empathetic approach, with a commitment to delivering high-quality care and support.
