Person Specification

Qualifications

  • Essential Criteria: Numeracy and Literacy - GCSE / A level or equivalent standard
  • Desirable Criteria: NVQ level 3 in Health/Social Care Management or equivalent

Experience

  • Essential Criteria: Familiarity with patient flow and support for complex discharge processes in an acute care environment
  • Desirable Criteria: Experience in assisting with the planning of patient discharges involving CHC and Social Services

Technical Skills Competencies

  • Essential Criteria: Basic IT skills, assessment, planning, and organizational skills
  • Desirable Criteria: Awareness of audits and knowledge of NHS computer systems, e.g., Ecamis, EPR

Knowledge

  • Essential Criteria: Demonstrated understanding of both simple and complex discharge processes from an acute care setting, including the Mental Capacity Act
  • Desirable Criteria: Knowledge of national guidelines and targets related to discharge
