- Develop Health Promotion Programmes: Develop, implement, and embed health promotion and well-being programmes that promote healthy lifestyles and prevent disease.
- Identify and Manage Risk: Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects to patients' health.
- Request and Process Pathology Services: Request pathology services as necessary and process pathology results as required, ensuring that all results are accurately recorded and communicated to patients.
- Maintain Accurate Clinical Records: Maintain accurate clinical records and ensure that read codes are used effectively, ensuring that all patient information is up-to-date and accurate.
- Participate in Service Delivery Initiatives: Participate in national and local initiatives to enhance service delivery and patient care, ensuring that all initiatives are implemented effectively and efficiently.
- Ensure Clinical Knowledge is Up-to-Date: Ensure that clinical knowledge is kept up-to-date and mandatory training requirements are met, ensuring that all staff are equipped to provide high-quality care.
- Prepare and Maintain Environments: Prepare and maintain environments and equipment before, during, and after patient care procedures/interventions, ensuring that all areas are clean and safe for patients.
- Undertake and Record Tasks: Undertake, record, and follow guidelines for the tasks for which you have received appropriate training and achieved competence, ensuring that all tasks are completed accurately and safely.
- Provide Support and Guidance: Provide support and guidance to healthcare assistants where called upon, ensuring that all staff are equipped to provide high-quality care.
- Participate in Immunisation Programmes: Participate in national immunisation programmes for adults and children with the appropriate training and competency, ensuring that all patients receive the highest level of care and attention.
- Comply with QOF and Enhanced Services: Comply with QOF, enhanced services, and medicines management initiatives to maintain practice high achievements, ensuring that all initiatives are implemented effectively and efficiently.
- Perform New Patient Health Checks: Perform new patient health checks, ensuring that all patients receive the highest level of care and attention.
