Job Title: Head of Community Nursing
Job Summary:
We are seeking an experienced and skilled Head of Community Nursing to join our team at Change Grow Live. As a key member of our clinical leadership team, you will be responsible for ensuring the delivery of high-quality clinical services to individuals who require treatment for their health needs related to addiction.
Key Responsibilities:
- Develop and implement integrated pathways for individuals with addiction-related health needs
- Ensure the clinical model is functioning effectively, efficiently, and in line with CGL policy and national guidance
- Implement and maintain an immunisation and vaccination scheme to combat blood-borne viruses and other infectious diseases
- Provide advice and information to professionals on BBVs and the care of individuals with drug and alcohol issues
- Work in partnership with stakeholders to promote Change Grow Live clinical services to commissioners and health and wellbeing boards
- Contribute to the overall performance of the service to achieve contractual output targets
- Ensure all clinical services are delivered to a high standard in line with national guidance, CGL policies, procedures, and best practice
- Manage the clinical aspects of new developments within the service
- Ensure all legislative requirements are explained to employees, particularly those governing the clinical and social care/community field
- Engage in proactive line management and clinical supervision, and maintain a current knowledge of appropriate interventions
- Effectively manage and develop nursing staff, and participate in core training programmes as required
- Spend time in other regions and localities to teach, develop, and share specialist knowledge with colleagues, projects, and teams
- Work with the lead doctor and/or non-medical prescriber to formulate, plan, and implement individual treatment programmes
- Undertake regular performance reviews, identify areas of risk, and take appropriate action
- Liaise with clinical/medical teams, commissioners, and clinical partners to ensure appropriate cover and a quality service
- Seek to improve personal performance, contribution, knowledge, and skills
Requirements:
To be successful in this role, you will need:
- Significant experience in community nursing and leadership
- Strong clinical knowledge and skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a team and build partnerships with stakeholders
- Strong problem-solving and analytical skills
- Ability to manage and develop staff
- Current knowledge of relevant legislation and policies
