Position Overview

Permanent Position: Community Nurse Band 5 - Community Clinic

NELFT is looking for passionate and motivated Registered Nurses to enhance our community health services in Havering. As a Community Nurse, you will provide comprehensive healthcare to patients, families, and caregivers in their homes, including residential care facilities and nurse-led clinics.

Main Responsibilities

Your role will involve delivering treatment, guidance, and education within your professional capabilities. Candidates must demonstrate a commitment to providing high-quality, evidence-based care. The ideal applicant will be a registered nurse (Part 1) and will collaborate with an integrated community healthcare team, working across various professional and organizational boundaries. You will liaise with general practitioners, other healthcare professionals, and both statutory and voluntary organizations to deliver personalized care under the supervision of a District Nurse.

Essential Qualifications

  • NVQ level 2 or equivalent in literacy and numeracy
  • Current NMC R/N or equivalent first-level nursing qualification
  • Evidence of post-registration education and training

Desirable Qualifications

  • Mentorship qualification
  • Relevant post-registration courses in wound management, diabetes care, and continence assessment
  • Nurse Prescriber (V150)

Experience Required

  • Experience collaborating with multidisciplinary teams
  • Ability to work autonomously and as part of a team
  • Experience in delivering care packages involving multiple agencies
  • Reflective practice experience

Knowledge and Skills

  • Understanding of NHS priorities and clinical governance
  • Familiarity with the NMC Code of Professional Conduct
  • Ability to initiate patient assessments and care planning
  • Capacity to engage patients and caregivers as partners in care
