About the Role

We are seeking a caring and motivated Support Practitioner to join our team at Cornerstone Community Care. As a Support Practitioner, you will play a vital role in supporting individuals with all aspects of daily living, including cooking, support to activities and appointments, managing personal finances, assisting with medication and personal care.

Key Responsibilities

  • Support individuals with daily living tasks, such as cooking and personal care
  • Assist with activities and appointments, including hobbies and interests
  • Manage personal finances and assist with medication
  • Support individuals to maintain their own homes and be an active member of their local community

Requirements

  • A caring and compassionate attitude and approach
  • The ability to work well in a team and on your own initiative
  • Good communication skills
  • A positive attitude with a willingness to learn
  • Confidence in the use of computers
