Job Title: Community Nurse - Transformation Unit

Job Summary:

We are seeking a highly motivated and experienced Community Nurse to join our Transformation Unit. The successful candidate will be responsible for providing high-quality care to patients in their own homes, working closely with other members of the multidisciplinary team to achieve the best outcomes for patients.

Main Responsibilities:

  • Assess and evaluate the health needs of patients, particularly in the areas of palliative care, wound management, diabetes, and other long-term conditions.
  • Develop and implement individualized care plans to promote health, well-being, and independence for patients.
  • Work collaboratively with other healthcare professionals to provide comprehensive care to patients.
  • Conduct regular reviews and assessments to ensure the effectiveness of care plans and make adjustments as needed.

Requirements:

  • Registered Nurse Level 1 (Adult) qualification.
  • Mentor and assessor qualification or equivalent.
  • Evidence of continued professional development relevant to the role.
  • Experience of working in a multi-disciplinary team setting.
  • Experience in Community Nursing.

Skills and Knowledge:

  • Ability to demonstrate sound knowledge and application of up-to-date evidence-based clinical practice.
  • Effective time management.
  • Flexible, ability to work across sites.

Personal Qualities:

  • Ability to work under pressure.
  • Effective Team Member.

Other Requirements:

  • Independently mobile - requires a car.
  • Willing to work weekends and Bank holidays.
  • Ability to work across various sites.
