As a Healthcare Case Management Specialist, you will be recognized as a knowledgeable resource and support for your team, expected to:

Purpose of the Role:
The aim of this position is to assist the organization in providing a comprehensive and effective service. Case Managers will serve as role models, fostering a supportive environment that empowers staff to contribute to the high standards of care delivery. The ideal candidate will present themselves professionally, exhibit reliability, maintain a customer-focused approach, and thrive in a dynamic, client-centered setting.

Key Responsibilities:

  • Maintain ongoing responsibility for establishing and monitoring care standards and quality for service users.
  • Share responsibility for a caseload of complex clients, managing their care packages holistically in collaboration with the designated Nurse Case Manager.
  • Collaborate with recruitment, training, and scheduling teams to ensure adequate safe staffing levels for our clients.
  • Develop, review, and implement client care plans and risk assessments in accordance with regulatory standards and best practices, while also enhancing the skills of non-qualified staff.
  • Coordinate and lead the initiation and continuity of all assigned complex care packages, engaging with clients and their professional networks, participating in multidisciplinary teams, and ensuring clinical safety.
  • Engage in networking and proactively partner with Clinical Commissioning Groups and Social Services, attending case conferences and multidisciplinary meetings as necessary.
  • Work in tandem with the Nurse Case Manager to ensure the ongoing competency of healthcare workers, providing non-clinical observations, supervisions, and support sessions as needed.
  • Foster continuous quality improvement through client and third-party feedback, reflecting on personal and team performance, and collecting supporting documentation.
  • Cultivate a culture of person-centered care within the community and throughout Routes Healthcare.
  • Ensure high standards of documentation and record-keeping in compliance with national legislation and local standards.
  • Conduct comprehensive client reviews at regular intervals, adapting to client needs.
  • Perform regular audits of records and daily evaluations, implementing necessary actions and reporting trends to management.
  • Participate in meetings aimed at enhancing practice and sharing information across the organization; assist in developing Standard Operating Procedures and implementing evidence-based policies.
  • Prepare case studies as needed to promote Routes Healthcare or support tender processes.
  • Inform management of any issues that may negatively affect care quality or health and safety.
  • Identify opportunities for service development by ensuring effective systems are in place.
  • Ensure individual and team performance indicators are met.
  • Manage complaints in accordance with internal policies, prioritizing safeguarding concerns appropriately.
  • Undertake additional tasks and projects as directed by management.
