About the Role

We are seeking an experienced Children's Nurse to join our team as a Lead Nurse, Children's Continuing Care Specialist. This is an exciting opportunity to work with a progressive and friendly team, assessing and case managing the healthcare needs of children and young people with complex physical health needs.

Key Responsibilities
  • Collate and undertake comprehensive and coordinated assessments for decision-making for Continuing Health Care with children and families within health, social, and education care.
  • Promote Personal Health Budgets, ensuring integration into Children's Continuing Care process.
  • Attend decision-making panels to present own cases and provide clinical information.
  • Case manage children eligible for Continuing Care, ensuring timely reviews are conducted and packages continue to be effectively commissioned and are cost-effective.
  • Develop strong working relationships with partner agencies, including Acute Hospital Trusts, local authorities, schools, the Independent Sector, and others.
  • Have a forward-thinking approach.
  • Oversee the service in the absence of the Team Leader.
