Job Title: Advanced Clinical Practitioner Lead

We are seeking an experienced Advanced Clinical Practitioner to lead our Urgent Community Response Team. The successful candidate will be responsible for managing clinical care in partnership with individuals, families, and carers, and will work with a high degree of autonomy and complex decision-making.

About the Role

The post holder will be required to work with a high degree of autonomy and complex decision-making underpinned by a master's level award that encompasses the four pillars of clinical practice, leadership and management, education, and research. They will manage clinical care in partnership with individuals, families, and carers, which will include the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people's experience and improve outcomes.

Key Responsibilities
  • Manage clinical care in partnership with individuals, families, and carers
  • Work with a high degree of autonomy and complex decision-making
  • Develop and implement innovative solutions to enhance people's experience and improve outcomes
  • Collaborate with multidisciplinary teams to provide high-quality patient care
  • Contribute to the development of clinical policies and procedures
Requirements
  • Health Education England accredited Advanced Clinical Practice full masters
  • Maintained a portfolio of ongoing evidence or work-based learning
  • Independent non-medical prescribing or pharmacology level 7 profession relevant qualification
  • Able to demonstrate a good command of the English Language both written and verbal
  • Computer Literate in both data entry and communicating via IT
