About the RoleThe Community Children's Nursing (CCN) team in Southampton consists of experienced nurses who work collaboratively with children, young people, families, and the wider multi-disciplinary team to ensure care is provided as close to home as possible. We provide holistic, family-centered care to children and young people with a range of nursing needs, including acute and short-term conditions, long-term conditions, disabilities, complex health conditions, and life-limiting illnesses.
Key Responsibilities- Manage a caseload of children and young people with nursing needs, providing specialized, high-quality skilled nursing care, considering the physical, psychological, spiritual, and social needs of each individual.
- Develop, implement, and evaluate individualized programs of care, working closely with families and the multi-disciplinary team.
- Provide supervision and monitoring of junior staff, supporting their development and growth within the team.
- Work collaboratively with families to provide emotional support and specialist clinical care, sensitive to the changing and ongoing needs of each child and their family.
- Teach families and carers to carry out specific complex nursing care and procedures, enabling them to feel confident and competent in caring for their child.
- Participate fully in the on-call rota, providing end-of-life care out of hours as required.
- Demonstrate excellence in clinical practice, keeping up-to-date with new developments in nursing care and treatments, ensuring practice is evidence-based and utilizing current research.
