Job Summary

We are seeking a skilled and compassionate Care Coordinator to join our team in Gloucester. As a Care Coordinator, you will play a vital role in supporting our clients with complex care needs, ensuring their safety and well-being at all times.

Key Responsibilities

  • Provide high-quality care and support to clients with complex care needs
  • Develop and implement personalized care plans to meet the unique needs of each client
  • Work collaboratively with our multidisciplinary team to ensure seamless care delivery
  • Monitor and report any changes in client condition or needs to the care team
  • Contribute to the development and implementation of new care initiatives and programs

Requirements

  • Experience working in a care setting, preferably with complex care clients
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work a variety of shifts, including evenings and weekends
