About the Role

We are seeking a highly skilled and experienced Clinical Triage Specialist to join our Integrated Care Coordination Team at Midlands Partnership NHS Foundation Trust. As a key member of our team, you will play a vital role in ensuring that patients receive high-quality, timely, and effective care.

Main Responsibilities
  • Provide clinical triage assessment and support to patients, healthcare professionals, and other stakeholders.
  • Work collaboratively with the Clinical Team Lead to develop and implement effective triage protocols and procedures.
  • Contribute to the development and maintenance of our clinical triage portal and telephone line.
  • Participate in multidisciplinary team meetings and contribute to the development of care plans and strategies.
  • Provide education and training to healthcare professionals and other stakeholders on clinical triage and related topics.
Requirements
  • Qualified nurse with current NMC registration (Adult)
  • Working towards or willingness to undertake a Physical Health Assessment Module
  • Degree in Paramedic Science or equivalent qualification
  • Evidence of continual professional development.
Desirable Qualifications and Experience
  • Extended clinical course e.g. care of the Diabetic, palliative care, continence, mentorship.
  • Triage Qualification / Course
  • Community experience
  • Modules at Masters Level
Key Skills and Abilities
  • Highly developed IT skills, including an advanced level of experience using Health IT Systems, Outlook, Microsoft Teams and Microsoft Word.
  • Understanding of Information Governance, Data Protection, NMC standards and safeguarding vulnerable people.
  • Experience of dealing with, and implementing learning from complaints and incidents.
