Job Description

Remedicare is seeking a highly skilled and dedicated SEN Outreach Support Worker to join our team. As an Outreach Support Worker, you will play a vital role in providing support to children and their families who are at risk of permanent exclusion.

Key Responsibilities
  • Work with schools to provide support for children who are at risk of permanent exclusion, either in a group or one-to-one setting.
  • Provide outreach support to students who are unable to access education, identifying and assessing their needs and evaluating the impact of interventions.
  • Be aware of the services available in the local area and ensure that families in need are supported to access these services.
Requirements
  • Contribute to risk awareness and raise issues where appropriate.
  • Assess and manage risk associated with assigned cases/service delivery to ensure safeguarding of service users.
  • Monitor, manage, and deliver support plans in specified service areas.
Working with Others
  • Liaise and work in partnership with key leads for other activities that may impact on own areas of responsibilities.
  • Contribute to the induction of new staff.
