Position Overview

As a Community Care Specialist, you will play a vital role in enhancing the quality of life for individuals within the community. This position entails working collaboratively with a multidisciplinary team to deliver comprehensive support and care.

Key Responsibilities

  1. Conduct assessments to identify the care needs of individuals and develop tailored support plans.
  2. Collaborate with families and caregivers to ensure a holistic approach to care.
  3. Engage with various health and social care professionals to coordinate services effectively.
  4. Maintain up-to-date knowledge of relevant legislation and best practices in social care.
  5. Utilize excellent communication and analytical skills to manage caseloads efficiently.

Essential Qualifications

A recognized qualification in social work or equivalent is mandatory, along with registration with the appropriate regulatory body. Experience in health or social care settings is crucial, with a focus on assessment and care management.

Professional Skills

The ideal candidate will possess strong interpersonal skills, the ability to work independently, and a commitment to evidence-based practice. Knowledge of Self-Directed Support principles is advantageous.

Additional Requirements

A valid driving license is necessary for this role. Consideration will be given to candidates with disabilities that may affect their ability to hold a driving license.
