About the Role
We are seeking a highly skilled and dedicated Registered Mental Health Nurse to join our Community Mental Health Team in the Black Country. As a key member of our team, you will be responsible for assessing and formulating individual interventions, plans, and outcome measures that promote recovery for adults with mental health difficulties.
Key Responsibilities
- Assess and formulate individual interventions, plans, and outcome measures that promote recovery for adults with mental health difficulties.
- Work collaboratively with multidisciplinary teams to provide comprehensive care and support to service users.
- Develop and maintain effective relationships with service users, carers, and other stakeholders to ensure seamless care coordination.
- Contribute to the development and implementation of care plans that promote recovery, social inclusion, and empowerment.
- Participate in regular supervision and professional development activities to maintain and enhance clinical skills and knowledge.
