About the Role

We are seeking an experienced and dynamic Senior Nurse Practitioner to join our Community Rapid Response and Intermediate Care Team. As a key member of our team, you will play a vital role in delivering high-quality nursing care to patients in their own homes, promoting independence and reducing hospital admissions.

Main Responsibilities
  • Provide acute nursing assessment and interventions in patients' own homes
  • Ensure patients are referred onwards to community services as appropriate
  • Work collaboratively with multidisciplinary teams to deliver patient-centred care
  • Develop and implement care plans to meet individual patient needs
  • Contribute to the development of the service, identifying opportunities for improvement and implementing changes
Requirements
  • Registered Nurse with a passion for community nursing
  • Experience of working in a community or intermediate care setting
  • Strong leadership and communication skills
  • Ability to work autonomously and as part of a team
  • Up-to-date knowledge of clinical governance and quality improvement
