Job Summary:
We are seeking a highly skilled and experienced professional to lead our Community Care Services team. As Head of Community Care Services, you will be responsible for providing strategic leadership and operational management to our Community Care Services, including Integrated Community Teams, Hospital Discharge Teams, Community Inpatient Services, and Specialist Inpatient and Outreach Services.
Key Responsibilities:
- Provide strategic leadership and direction to the Community Care Services team
- Develop and implement effective operational plans to achieve service goals and objectives
- Lead and manage a team of professionals to deliver high-quality care and services to our patients and clients
- Collaborate with other stakeholders, including NHS Fife, Fife Council, and the independent and voluntary sectors, to develop and implement integrated care pathways
- Ensure compliance with relevant policies, procedures, and legislation
Requirements:
- Proven experience in leading and managing complex services and teams
- Strong strategic leadership and operational management skills
- Excellent communication and interpersonal skills
- Ability to work collaboratively with other stakeholders to achieve shared goals and objectives
- Knowledge of the national policy landscape for community care services
