About the Role

We are seeking a highly skilled and experienced Clinical Pharmacist to join our Respiratory Medicine team at Guy's and St Thomas' NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality clinical pharmacy services to adult respiratory patients.

Key Responsibilities
  • Provide safe, efficient, and high-quality clinical pharmacy services to adult respiratory patients under the care of the Cystic Fibrosis and Host Defence teams.
  • Work closely with the Lead Pharmacist to develop the pharmacy service to respiratory patients and support the development of protocols and guidelines.
  • Develop and maintain specialist knowledge in medicines used within respiratory medicine.
  • Participate in multidisciplinary ward rounds and committees as agreed with the Lead Pharmacist.
  • Monitor, audit, and provide specialist support for medicines-related risk and collaborate with the multidisciplinary team in reducing and managing such risk.
  • Deliver safe, efficient, and cost-effective use of medicines within respiratory medicine by providing education and training, procedures, protocols, and medicines usage information.
  • Support the Homecare service for respiratory patients, including screening and prescribing of prescriptions.
  • Undertake patient consultations in outpatient clinics, conducting reviews, identifying adherence problems, making medicines plans using clinical guidelines, and identifying complex referrals for further input by Specialist pharmacists, Consultants, and other relevant MDT members.
  • Provide comprehensive medication review to Cystic Fibrosis patients as part of the annual review process.
