We are seeking a compassionate and dedicated individual to join our award-winning team as a Team Leader at Avery Healthcare.
Key Responsibilities- Lead in Person-Centred Care Planning: Implement, monitor, and undertake monthly evaluations, complete care plan audits, develop action plans, and be responsible for completing the action plans.
- Support Resident Transitions: Assist in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
- Monitor Resident Well-being: Lead with monitoring the well-being of each resident, being aware of any change impacting care and care planning, and ensuring that changing needs are met and written records are amended to reflect any variation.
- Practice Safe Systems of Work: Assess risks and have due regard for personal safety and the safety of residents, visitors, and staff.

Our ideal candidate must:
- Have an NVQ 3 or equivalent in care for care homes.
- Two years' experience working in a senior position with the resident group.
- Have the ability to communicate effectively both verbally and in writing.
- Demonstrate the ability to lead staff and act responsibly in the absence of the manager.
