Main Job Responsibilities

We are seeking a compassionate and motivated Respiratory Nurse Specialist to join our team at Leeds Community Healthcare NHS Trust. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality care to patients with chronic respiratory conditions, primarily COPD, in their homes and clinics.

You will work closely with colleagues in local hospitals to facilitate hospital discharges and support admission avoidance, chronic disease management, and pulmonary rehabilitation. Your role will involve delivering care in patient homes, making decisions, and working independently with the support of the wider team.

We offer full training and support to ensure your success in this role. You will primarily work Monday to Friday from 8:30am to 4:30pm, with occasional weekend shifts. Due to the nature of the role, it is essential that you can travel independently across the city and have access to a suitable vehicle for business purposes.
