As a Care Coordination Specialist, you will utilize your practical and procedural expertise, along with analytical and decision-making skills, to interpret various situations and address diverse challenges. You will be responsible for determining the appropriate methods and timing for executing specific tasks and will operate with a degree of independence.
Your role will involve sourcing placements in Care Homes, which necessitates engaging in discussions and negotiations with Social Care and Health personnel, service users and their representatives, as well as registered providers. A thorough understanding, analysis, and evaluation of the Care Home market within the region will be essential.
Every day will present unique challenges as you work with a diverse range of individuals with varying needs.
