About the Role:We are seeking a compassionate and skilled Licensed Practical Nurse (LPN/LVN) to join our team of healthcare professionals. As a Private Duty Nurse, you will work one-on-one with patients, providing personalized care and support to help them achieve their goals and improve their quality of life.
Responsibilities:- Implement and update patient care plans in collaboration with the healthcare team
- Provide skilled nursing care, including wound care, medication management, and vital sign monitoring
- Assist patients with daily living activities, such as bathing, dressing, and grooming
- Document patient progress and communicate with the healthcare team to ensure seamless care
- Participate in patient and family education to promote health and wellness
- Current, unencumbered LPN/LVN license in the state of practice
- Current CPR certification
- One year of experience in pediatrics or home health care
- Ability to work independently and as part of a team
- Strong communication and interpersonal skills
