About the Role

We are seeking a skilled and dedicated Community Nurse to join our team at Hertfordshire Community NHS Trust. As a Community Nurse, you will play a vital role in providing high-quality patient care and promoting health and wellbeing in the community.

Key Responsibilities
  • Assess, plan, implement, and evaluate individualized nursing care to patients living in the community.
  • Take responsibility for managing a caseload and working collaboratively with other healthcare professionals to achieve optimal patient outcomes.
  • Develop and maintain strong relationships with patients, families, and carers to ensure person-centered care.
  • Contribute to health promotion, education, and training initiatives to support the health and wellbeing of the community.

Person Specification

  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and carers.
