We are seeking a skilled Care Coordinator to join our dedicated team at Apollo Home Healthcare. As a Care Coordinator, you will play a key role in the continuous delivery of care to our service users.
Key Responsibilities:
- Manage a caseload of care packages and understand the varied nature and fast-pace of the role.
- Apply communication and relationship building skills to effectively manage rota's for care teams.
- Respond efficiently to any day-to-day changes and act as the first point of contact for service users, their families, and other healthcare professionals.
- Work well under pressure, using initiative and thinking on your feet.
Requirements:
- Experience in care coordination, preferably in an office-based setting.
- Ability to travel across the East and South East region, with a valid driving license and access to a vehicle.
- Commitment to supporting the out of hours on call service, covered on a 1 in 5 rota basis.
What We Offer:
- Competitive salary between £29,560- £34,560 per annum, including payment for on-call.
- Bonus scheme of up to £2,000 per annum.
- 25 Days Annual Leave, plus Bank Holidays.
