Care Home Night Nurse Job Description

We are seeking a highly skilled and compassionate Care Home Night Nurse to join our team at Gloucestershire Health and Care NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality patient care and support to individuals with mental health needs.

Clinical Responsibilities
  • Develop and implement individualized care plans in collaboration with patients, carers, and other healthcare professionals.
  • Conduct regular mental and behavioral state examinations, and clinical risk assessments to ensure patient safety and well-being.
  • Provide emotional support and guidance to patients, promoting independence and self-management of their mental health.
  • Liaise with other healthcare professionals, including doctors, therapists, and social workers, to ensure comprehensive patient care.
  • Participate in multidisciplinary team meetings to discuss patient progress, share knowledge, and develop care plans.
Documentation and Communication
  • Maintain accurate and up-to-date patient records, including notes, statistics, and reports.
  • Contribute to the preparation of reports for Mental Health Act Tribunals and other relevant meetings.
  • Communicate effectively with patients, carers, and other healthcare professionals, using clear and concise language.
  • Ensure that patient information is shared securely and confidentially, in accordance with Trust policies and procedures.
Leadership and Management
  • Support and supervise junior staff members, providing guidance and mentorship to ensure their professional development.
  • Contribute to service development and quality initiatives, using audit and policy development to improve patient care.
  • Participate in training and education sessions, sharing knowledge and expertise with other healthcare professionals.
  • Take ownership of patient care, making decisions and taking actions to ensure patient safety and well-being.
