Job Summary:
We are seeking a highly skilled and motivated Primary Care Mental Health Nurse to join our team. As a Primary Care Mental Health Nurse, you will work closely with GPs, Practice Managers, and other healthcare professionals to provide high-quality mental health assessments and interventions in a primary care setting.
Key Responsibilities:
- Provide holistic mental health assessments and evidence-based interventions to individuals with mild to moderate mental health difficulties.
- Work collaboratively with GPs, Practice Managers, and other healthcare professionals to develop and implement care plans.
- Interface with secondary care and community-based services to ensure seamless care coordination.
- Provide professional leadership and supervision to junior staff members.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
- Registered Mental Health Nurse with a valid practicing certificate.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of mental health legislation and policies.
- Non-medical prescribing qualification an advantage.
