About the Role
This unique role is part of a collaborative project with The Stroke Association, focused on supporting patients recovering from stroke. You will play a vital part in helping individuals transition smoothly back into their communities after rehabilitation at the Sussex Rehab Centre.
Your Responsibilities
- Develop and implement personalized rehabilitation programs for stroke patients, working under the guidance of qualified therapists.
- Provide ongoing support and rehabilitation sessions both before discharge from the Sussex Rehabilitation Centre and in community settings afterward.
- Collaborate closely with a multidisciplinary team to deliver an effective transitional care model.
- Work as part of the wider Sussex Stroke Network, fostering a collaborative approach to rehabilitation.
- Contribute to project evaluation by maintaining accurate records and administrative duties.
About You
We are seeking individuals with a passion for supporting stroke survivors. Your experience working in healthcare, combined with your understanding of multidisciplinary teamwork, will be invaluable in this role.
