We are seeking an experienced and compassionate Associate Haematology Specialist Nurse to join our Hospice at Home team. As a key member of our interdisciplinary team, you will provide expert clinical assessment, monitoring, and management of complex palliative care needs for patients receiving care in their own homes.
Key Responsibilities:- Develop and implement individualised care plans in collaboration with patients, families, and other healthcare professionals, ensuring holistic support and symptom management.
- Offer emotional support, education, and guidance to patients and their families, empowering them to make informed decisions about their care and treatment preferences.
- Coordinate care delivery with members of the Hospice at Home team, including healthcare assistants, social workers, therapists, and volunteers, ensuring seamless continuity of care.
- Provide education and training to patients, families, and caregivers on symptom management, medication administration, and other aspects of palliative and end-of-life care.
- Maintain accurate and timely documentation of patient assessments, care plans, interventions, and outcomes in accordance with hospice policies and regulatory standards.
