We are seeking an experienced and skilled Advanced Nurse Practitioner to join our team at the Shropshire Rural Alliance PCN. The successful candidate will work autonomously, providing high-quality patient-centered care to patients in care homes and the community.
Main Responsibilities:- Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition.
- Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of care.
- Assess and triage patients, manage undifferentiated, undiagnosed conditions, and provide definitive treatment or make necessary referrals to other members of the primary care team.
- At least 3 years' experience as an Advanced Nurse Practitioner.
- Experience in the acute examination of adults and children.
- Management of common minor injuries/illnesses.
- Experience as an autonomous practitioner.
- Able to work under pressure and manage varying workloads and changing demands.
- Master's Degree in Advanced Clinical Practice or equivalent.
- Current Registered Nurse (NMC).
- An understanding, acceptance, and adherence to the need for strict confidentiality.
The successful candidate will be expected to work across all PCN sites, with flexibility to work varying hours and commit to team working.
