Community Hospice & Palliative Care is seeking a skilled Hospice APRN to join our team. As a Collaborative Care Specialist, you will work closely with physicians and other healthcare professionals to deliver high-quality patient care and services in a variety of settings.
Key Responsibilities- Collaborative Care: Work in collaboration with physicians and other healthcare team members to deliver patient care and related health services.
- Patient Assessments: Perform medical examinations and evaluations, diagnose, treat, and follow up with patients and families.
- Health Condition Management: Assist with diagnosis, treatment, and management of acute and chronic health conditions.
- Diagnostic Testing: Order, perform, and interpret laboratory and radiology tests within the scope of professional practice.
- Medication Management: Write orders for informed consents, diagnostic procedures, and treatments. Prescribe medications and administer as prescribed.
- Communication and Collaboration: Consult with physicians or designees as needed, inform attending physicians of services provided, and collaborate with other physicians as needed.
- Documentation and Record-Keeping: Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement of services provided.
- Compliance and Reimbursement: Comply with applicable laws and regulations and initiate reimbursement for services rendered on a timely basis.
- Confidentiality and Education: Ensure provision of informed consent for CPR and DNR, recommend medications as required, and adhere to the practice of confidentiality.
- Team Collaboration and Education: Provide educational sessions to advance hospice care among internal staff, medical community, and community as a whole.
- On-Call Coverage and Scheduling: Participate in on-call coverage and assist in establishing schedules.
- Interdisciplinary Team Meetings: Participate in regular interdisciplinary team meetings to review active cases.
- Internal Controls and Coordination: Adhere to internal controls established for the department and coordinate Inpatient APRN work assignments.
- Staff Communication and Updates: Communicate potential assignment changes based on daily census, review and coordinate Inpatient APRN PTO requests, and prepare group texts for proper communication.
- Staff Support and Development: Update and consult with the Chief Medical Officer on APRN staff updates when appropriate and act as a point person for Inpatient APRN staff during emergency alert status.
- Education: Graduate, advanced education with Master's degree.
- Experience: Minimum of two (2) years nursing experience. Experience in hospice palliative care preferred.
- Licensure: Valid State of Florida Driver's license with driving record acceptable to Community Hospice insurance carrier. Advanced Practice Nurse or PA licensed in the State of Florida.
