**About Us:** Adult Social Care is a leading provider of social care services, dedicated to delivering high-quality care and support to individuals in need.

**Job Summary:** We are seeking an experienced Telecare Coordinator to join our team, responsible for providing support and guidance to multi-disciplinary teams, leading on delivering training, and carrying out Telecare assessments and reviews.

**Key Responsibilities:**

  • Provide support and guidance to multi-disciplinary teams to support staff training and development across health and social services.
  • Lead on delivering training for staff to prescribe Telecare following an agreed training course, organizing and providing appropriate training to staff, and promoting Telecare at public events.
  • Carry out Telecare assessments and reviews where required to identify appropriate service provision to meet any identified Health or Social Care need.
  • Work closely with health and social care professionals to promote Telecare within the context of enablement and personalization.

**Requirements:**

  • Previous experience working in Telecare Service
  • Flexible occasional hours
  • Enhanced DBS

**Salary:** £25 per hour (umbrella rate)
