Job Overview

We are seeking a highly motivated and experienced Senior Staff Nurse to join our Unplanned Care team. As a key member of our team, you will be responsible for providing high-quality patient care in a dynamic community nursing setting.

Main Responsibilities
  • Provide clinical visits to patients in their own homes, assessing, planning, managing, and evaluating their care needs.
  • Support the clinical lead with daily leadership and decision-making.
  • Liaise with the multidisciplinary team to ensure seamless care delivery.
  • Develop and implement care plans that meet the individual needs of patients.
  • Collaborate with other healthcare professionals to achieve the best possible outcomes for patients.
Requirements
  • NMC current registration.
  • RGN/RN professional qualifications.
  • Ability to delegate work and demonstrate effective management and development of staff.
  • Understanding of the role of the District Nursing service.
  • Ability to use Microsoft Office and Outlook.
Desirable Criteria
  • Previous experience of community healthcare (District Nursing).
  • Understanding and/or experience of managing patients with long-term conditions.
  • Knowledge of diabetes management, wound care, and leg ulcer management.
  • Knowledge of SystmOne.
