Role Overview

The Lead Investigator will play a pivotal role within the Quality Assurance Team, tasked with engaging key stakeholders both internally and externally to cultivate and implement exemplary investigative practices throughout the organization.

This position focuses on establishing and integrating a root cause analysis methodology to guarantee that high-quality investigations are consistently performed, with shared learning outcomes when challenges arise.


Key Responsibilities:

  • Serve as the principal authority for all investigative matters, providing support to investigators and ensuring that inquiries (including disciplinary, safeguarding, financial, grievance, and serious incidents) are conducted in alignment with established policies and procedures, collaborating effectively with stakeholders.
  • Lead serious incident investigations and manage complaints, offering guidance and support to team members.
  • Collaborate closely with the Quality Programme Manager on complaints, ensuring timely acknowledgment, thorough investigation, reporting of outcomes, and implementation of corrective actions.
  • Assist in the development and maintenance of best practices across the organization.
  • Ensure that investigations are well-defined, conducted within set timelines, and that reports are clear, impartial, and identify systemic and individual factors contributing to issues, along with organizational learning.
Candidate Profile
To qualify for the Lead Investigator role, candidates must possess the following qualifications:

- Proven experience in investigating serious incidents, complaints, and safeguarding matters within a large and complex organization.

Proficiency in conducting investigations utilizing root cause analysis methodologies. Background in investigations within the health and social care sector or relevant transferable experience. Comprehensive understanding of the regulatory and compliance frameworks applicable to third sector social care organizations. Exceptional skills in report writing. In-depth knowledge of safety and investigative procedures, particularly in root cause analysis. About the Organization
