About the Role

We are seeking a dedicated and compassionate Support Worker to join our team in West Wick. As a Support Worker, you will play a vital role in enabling individuals to live fulfilling lives, building relationships, and promoting independence.

Key Responsibilities

  • Empower individuals to make informed decisions and take control of their lives
  • Develop and implement daily living skills, promoting independence and confidence
  • Support individuals to access their communities, fostering socialization, employment, and friendships
  • Contribute to and develop individual care plans, ensuring tailored support
  • Communicate effectively with individuals, families, and external professionals, promoting a collaborative approach
  • Maintain the health needs of individuals, ensuring their well-being
