Job Overview

We are seeking a highly motivated and compassionate Support Worker to join our team at The Action Group. As a Support Worker, you will play a vital role in providing a caring, efficient, and effective 24/7 support service to individuals who use our services.

Main Responsibilities:

  • Practice the TAG Key Worker system, including organizing support plans and case reviews, ensuring effective handovers between staff, and maintaining records and case files.
  • Promote the physical and emotional wellbeing of individuals, supporting them to maintain acceptable and adequate levels of self-care, carrying out risk assessments, and more.
  • Develop meaningful and trusting relationships with individuals, helping them gain insight into their own behavior, attitudes, and reactions.
  • Encourage independence in daily living activities, supporting individuals only when necessary, and promoting their hobbies, interests, and activities.
  • Directly assist with personal care as required, work with individuals to maximize their ability to communicate effectively, and advocate on their behalf when needed.
  • Work with relatives and carers, arranging support plans, dealing with complaints and frustrations, and ensuring they have a significant role in the individual's life.
  • Find out about local services, amenities, and organizations available to individuals, and work with both individuals and groups as needed.
  • Participate in flexible shift work patterns, including regular working outwith weekday office hours, and undertake 'sleepover' or 'waking night' duties where required.
